Since my AE and AM and underwriter assistant dont want to get back to me I need you my friends to help me out on this one....For those who know what Im talking about is this for a refiance or purchase? Im going in circles with the underwriter about this form. Im refinancing this ladys house and they are asking for this form to be in the file. (last condition to doc my file!)

It’s not a form, you have to go to both web-sites and do a name search on every party involved in the transaction ie. appraiser, title ect. Here's one web-site, but i don't have the other one with me

Thanks much for the info so why are they giving us a form to fill out and its asking for those items as well as seller and buyer info. to fax this form to xxx number. We did that and they say its not correct im pulling my hair out over here....

Hmm, is the form an addendum to the mccaw, and asks for a list of all parties associated with the transaction? All the lenders that I deal with have you pull the info yourself maybe your lender just wants a list and then will do it themselves.

Maybe the lender that you're usng has their own form on the web-site if no you could probably cut and paste this form onto your letter head

this is what its has on the top of the form

LDP/GSA - Attachment to the mccaw.

We filled out the form uploaded it to the underwriter so frustrating. Im here in ca underwriter is CO if it was up to me i would personally walk into that office and get what is needed to be done this is crazy.

that's weird i've never had to fll out the actual form for them useually i just go to the site and run all of the names.

I have closed loans with them before FHA never have had this problem I finally got off the phone with AM and she keeps asking for this form and I said where do i need to get it or what needs to be done and she doesnt even know what it is!!!! She said go to such and such website i think. blah blah. they need to stop hiring these temp agency employees!!! days waisted becuase of her! Unbelievable!
